CMS' Global and Professional Direct Contracting Model is getting a new name and look. Effective Jan. 1, 2023, the agency is replacing the GPDC model with the redesigned ACO REACH Model, intended to improve care coordination and outcomes for Medicare patients, especially those in underserved areas.

This webinar will outline the incentives and requirements laid out by the new ACO REACH model and share how organizations can leverage actionable interoperability to make them achievable. By encouraging healthcare providers and giving them tools to make it happen, unified care coordination can be realized.

Learning objectives:
  • ACO REACH model incentives and requirements — what's new and important
  • Actionable interoperability — what it means and why it matters
  • What effective care coordination looks like today, and what will be done to improve upon it
