HCM care models: How top nurses approach community engagement and collaboration

This webinar is on-demand and can be viewed at your convenience.

Ensuring optimal outcomes for patients with hypertrophic cardiomyopathy (HCM) — a common form of genetic heart disease — requires significant support both inside and outside of the clinical setting. What's more, promoting community awareness of this  undiagnosed condition can help to prevent complications like heart failure and financial burden down the line.

In this session, a panel of nurse leaders from health systems will discuss two HCM models of care that incorporate community engagement to support patient outcomes.

You will learn:
  • Pillars of community engagement for HCM patients
  • How HCM diagnoses and outcomes intersect with the social determinants of health
  • Strategies for education and awareness
