2:00 PM CT Imagine your ED is caring for an unresponsive patient, and you're not able to reach their family or primary care provider. The EHR shows an ED visit from over five years ago that is not pertinent to the patient's current condition, and there are no additional insights into what care the patient has received since that visit. How do you best provide care with these information gaps? Alternatively, imagine having a patient present to the ED with a myriad of chronic complaints with significant risk factors and comorbidities. The patient has been to 20 different hospitals in the last six months, and the EHR contains mountains of medical records.
