Rare diseases can be difficult to recognize, which often limit patients' access to necessary care and ultimately affects patient outcomes. Ensuring patients are connected to the most appropriate, effective treatment pathways starts with health systems' commitment to better identifying and diagnosing individuals with these conditions.

During this webinar, key findings from a recent survey of health system executives, including senior pharmacy, service line leaders and administrators, will be shared, followed by a panel discussion of how Pharmacy Leaders have implemented changes at their institutions to best meet the needs of patients with rare diseases.
