12:00 PM CT  Nearly half of U.S. hospitals now offer remote patient monitoring (RPM) services. The next challenge is turning those programs into better chronic disease management, stronger patient engagement and measurable outcomes. In fact, the Centers for Medicare and Medicaid Services (CMS) has signaled updates to the RPM reimbursement structure in 2027 to enhance the impact of RPM on patient outcomes. Remote patient monitoring can support organizations across fee-for-service, value-based care or mixed reimbursement models. But reimbursement alone is not what makes a program sustainable. Long-term success depends on selecting the right patients, designing proactive care workflows and focusing on clinical outcomes first.
