1:00 PM CT  Picture a cardiovascular patient moving from the ED to inpatient to ambulatory to home without a single dropped handoff — follow-up intact, the care team aligned, nothing left to chance in the gaps. For most health systems, that's not yet the reality. Transitions remain one of cardiovascular care's most persistent weak points, addressed department by department instead of as a connected pathway. But a proactive, pathway-based model can change that, turning reactive fixes into a coordinated strategy that strengthens follow-up and reduces operational burden. On Aug. 12, leaders from RWJBarnabas Health and Advocate Health will share what that shift requires.
