1:00 PM CT On January 1, 2027, the Centers for Medicare & Medicaid Services (CMS) will launch the Ambulatory Specialty Model (ASM)—a mandatory payment model that fundamentally redefines how cardiology practices manage heart failure (HF). Unlike previous voluntary value-based programs, ASM is not optional. General cardiology practices in selected regions will be automatically enrolled based on historical patient volume. Under ASM, CMS is shifting accountability from episodic, office visit-driven encounters to continuous, longitudinal patient management. This means financial and clinical success depends on what happens between clinical visits. Join leading cardiovascular executives, clinical directors, and value-based care experts for a practical roadmap on navigating the next 12 to 18 months.
