Denials continue to increase. - Providers say authorizations and incorrect information on claims are driving the rise in denials. - Providers want to prevent denials versus just managing them.   Sample findings:   - 38% of respondents acknowledged that 10% or more of claims are denied. - 46% of respondents say that missing or inaccurate data is one of the top three reasons for denials. - 65% of respondents say that submitting "clean" claims is more challenging than before the pandemic.
