The power of streamlined patient transfers — Lessons from University of Maryland Medical System

Timely and efficient patient transfers are key to achieving the best possible outcomes and ensuring patients receive the right level of care at the right time. Transfer center workflows can be cumbersome and inefficient when care teams rely on antiquated communication methods like faxes and pagers. This can lead to delays in care, longer than necessary lengths of stay, and suboptimal experiences for patients, families and care teams.

Clinical communication platforms can help accelerate patient transfers by facilitating secure, real-time communication between the care teams and the transfer center. The University of Maryland Access Center (UMAC) is modernizing its patient transfer workflow. In this session, Dr. Mark Sutherland, Medical Director of the Patient Placement Division at UMAC will discuss:
