Connecting Health Systems and Community-Based Organizations to Improve Outcomes

Successful health outcomes are often linked to social determinants of health. As health systems are pressured to reduce length of stay and prevent readmissions, there is an ever-increasing need for patients to successfully connect with social service organizations such as those providing housing and employment assistance. Enabling processes, including transparent data sharing between health systems and community-based organizations, not only helps create better outcomes for vulnerable populations, but it also facilitates more effective care transitions, improves chronic disease management and reduces hospital readmissions. In this session, we will discuss how the combination of artificial intelligence, advanced analytics, and cloud-based information sharing help improve community health processes and outcomes.
