Trinity Health & the AMA collaborate in the fight against heart disease

Tuesday, January 28th, 2020 | 11:30 am - 12:30 pm CST

Cardiovascular disease continues to be the leading cause of death in the U.S. Learn how Trinity Health is prioritizing prevention by implementing the AMA’s MAP BP Program – an evidence-based, continuous quality improvement program – achieving as much as a 12% absolute increase in blood pressure control rates at some locations in less than a year.

Offered as part of the AMA’s Health System Program suite, MAP is one of the many service improvement offerings available to large health systems. Trinity Health’s regional success using the program led to the incorporation of MAP as the organization’s system-wide approach to improving blood pressure control in 22 states.

Webinar attendees will learn:

  • How MAP (Measuring accurately, Acting rapidly and Partnering with patients) supports medical groups and health care systems in translating the latest evidence into practice to achieve measurable improvement
  • How to apply methods to reduce diagnostic uncertainty, clinical inertia and medication nonadherence
  • How to modify workflows to overcome challenges and leverage opportunities to implement quality improvement processes into clinical practice
  • How to prepare an operational plan for adoption and implementation of MAP as a population health management strategy
